When applying for the JPA Scholarship, applicants must prepare and upload/submit the following documents to complete their application. Missing documents may result in automatic rejection.

Personal Identification
- A copy of the applicant’s MyKad (Identity Card).
- A copy of the birth certificate.
- A copy of the parents’/guardians’ MyKad.
Academic Documents
- Certified copy of SPM / STPM / Matriculation / Foundation / Diploma results.
- Certified copy of latest exam transcripts (for undergraduates).
- Senate Confirmation Letter of Completion of Study (for graduates applying to postgraduate sponsorship).
- School/college/university leaving certificate (if applicable).
Admission & Course Documents
- University/College offer letter (local or overseas).
- Proof of course registration.
- Details of the approved program (course outline, duration, credit hours).
Financial & Family Information
- Proof of parents’/guardians’ income (salary slip, income tax statement, or statutory declaration).
- Statement of household income (especially for B40 category applicants).
- Guarantor information (parents/guardians or as required by JPA).
Additional Supporting Documents
- Passport copy (for overseas applicants).
- Curriculum Vitae (CV)/Resume (for postgraduate applicants).
- Proof of co-curricular achievements, certificates, or awards.
- Medical report (if required by specific scholarship programs).
- Marriage Approval Form (if applying for marriage permission during studies).
Confirmation & Reporting
- Slip Pengesahan Permohonan – Application confirmation slip.
- Slip Pengesahan Ujian Psikometrik – Proof of psychometric test completion.
- Supporting documents for Graduate Self-Report (degree certificate, exam transcript, employment proof) via mySELF JPA Portal.
Important Note:
- All documents must be certified true copies by the school principal, head of department, or a commissioner of oaths.
- Keep both hard copies and scanned copies (PDF format) ready before applying online at the official JPA portal.
